Adjustable threshold cap

I need the wooden gap height adjustment piece for the threshold of an exterior door. The attached photo only shows a short piece as it was rotten and deteriorated.

The Wooden piece is 1 3/8" wide and 36" long. The made-in screws are located at 4 1/2", 13 1/2", 22 1/2", and 31 1/2".

Your help would be greatly appreciated. Thanks

I'm sorry to say that this isn't something we can help with. I would recommend contacting a local carpenter to see if this is something they can fabricate for you.
