Could you please identify this part? I need to order two and replace, if possible. If you have the part, will it also come with some type of installation instructions?

Your pivot shoe looks like our 15-199. This is compatible with our Series 160 Coil Springs. For a new support, see our 15-205. Let us know if you have any questions.
